Scheduling & Telehealth

Please arrive 10 minutes early for check-in. Call if you’re running late; after 15 minutes we may need to reschedule. Cancellations with less than 24 hours’ notice may incur a fee.
Chronic Care & Referrals
-
Can you manage my chronic conditions?
Yes. We create a personalized care plan, review medications, and monitor labs. Remote BP/glucose programs are available for some conditions.
-
Do I need a referral to see a specialist?
Some insurance plans require referrals. We’ll coordinate after your visit; allow 3–5 business days unless urgent.
Forms & Records
-
Can you complete school/work/sports forms or FMLA paperwork?
Yes. Forms usually require a visit and up-to-date exam. Standard processing time is 3–5 business days; rush fees may apply.

How do I transfer medical records?
Sign a release in our office or via the patient portal. We can request records from your prior clinic or send your records to another provider.
Access, Privacy & Safety
-
Do you offer language support or accessibility accommodations?
Yes. Tell us if you need an interpreter, wheelchair access, or a chaperone—we’ll arrange it.
-
Is my information private?
Absolutely. We follow HIPAA and safeguard your health information. See our Notice of Privacy Practices.
-
What should I do in an emergency?
For chest pain, severe shortness of breath, stroke symptoms, or other emergencies, call 911 or go to the nearest ER. For urgent but non-emergency issues, call us.

Copays are due at check-in. After your insurer processes the claim, you may receive a bill for deductibles/co-insurance or non-covered services.
Prescriptions & Refills
-
How do I request a medication refill?
Use your patient portal or ask your pharmacy to send us an electronic request. Allow 2 business days. Some refills require a follow-up visit or lab work.

Do you prescribe controlled substances?
We follow strict guidelines and may require regular visits, treatment agreements, and monitoring. Not all controlled medications are prescribed.
Labs, Imaging & Results
-
Can I get labs at your clinic?
Most labs can be drawn on-site or at a partner lab. Bring your insurance card; coverage varies by plan.

How will I receive results?
Results are released to your patient portal with comments. We’ll call for any urgent findings. Please allow 2–5 business days for routine labs.

Do you offer X-rays or imaging?
We refer to local imaging centers. Your clinician will send the order and review results with you.
Preventive Care & Vaccines
-
How often should I have a physical?
Most adults benefit from an annual preventive visit that focuses on screening, risk reduction, and wellness—not the same as a sick visit.
-
Which vaccines do you provide?
Flu, COVID, Tdap, and others based on age/condition. Ask about travel or specialty vaccines.
After-Hours & On-Call
